Hmm... just to double check, have you looked at the entire list as I notice that the scroll bar for your banned IPs list is massive. If it's definitely not in that list then there is something particularly weird going on here.
I'm 99% sure that the only possible way to get that banned page is to have some sort of criteria match within the phpBB banning system so I can't imagine there is anything server side that has banned the IP and is redirecting to that page as it's dynamic.
